44a33126e0 wctc4xxp: Only capture commands once they are on the descriptor ring.
Eliminates some cases where there are duplicated packets in the capture if the
hardware descriptor ring was already full.

3446cdca8f wctc4xxp: Do not allow duplicated sequence numbers to be received for the channels.
Makes the channels themselves behave like the supervisor channel. This only
protects the driver in the case the commands were severly backed up, like when
there was high packet loss.

391ca2b7aa wctc4xxp: Do not need locks on the transcoder buffers.
They are sufficiently protected by the list locks. This also cleans up a case
where the tcp was unlocked after already completing it, which was corrupting the
list.

7168b87cb5 wctc4xxp: Always ack a response packet.
Even if it is duplicated or we don't have an outbound message waiting, we should
ack it so that the firmware does not keep trying to send it to the host.
Otherwise the firmware could get into situations where it was constantly
retrying to send packets for which it did not receive our previous ACK and
exhaust memory.

I was only seeing this on platforms were packets were going missing in the
stream, increasing the probability that the driver would miss early responses.

6c796d0774 wctc4xxp: Replace channel semaphore with channel mutex.
Clarifies that the semaphore was being used as a mutex. Mutexes are also more
efficient and allow better debugging checks.

9c65971863 wctc4xxp: Remove unused debug ioctl interface.
The ioctls for the debug network interface on the tc400b0 has not been used for
a long time. It is now gone.

This will also allow the sempahore set in the ioctl to be changed into a mutex
which provides enhanced debugging checks.

2ac2338247 wctc4xxp: Halt the card when an alert is received.
When an unsolicited alert is received, we'll flag the card as halted so that
commands will not be retried. This is because often times the firmware will no
longer process any commands in this state and the driver will hold processes in
the dstate while waiting to retry the commands.

This is a debugging aide in that it simplies unloading the driver if the card /
driver is currently in a failed state.

3096ffe955 wctc4xxp: Disable read-line and read-line-multiple PCI commands.
The read-line-multiple command was already disabled on the voicebus cards, which
use the same interface, in commit 4de462c3e0. This
does the same thing for the transcoder card and also disables the read line
command.

I've seen this change directly correlated to problems with the AN983 receiving
packets from the onboard DSP on some platforms.

039daca12e wctc4xxp: Make sure we call the pci_enable_mwi() function.
I've not seen this directly tied to any issue, but it's enabled on the voicebus
cards and so brings the wctc4xxp driver in line.

8256ba602e wctc4xxp: Trivial. Remove unused timer_list from struct tcb.
The timer_list member of the transcoder buffer has not been used for a long
time. Saves some space in the buffer in addition to removing any future
curiosity about what it is for.

040ba6f43a wctc4xxp: Ensure the descriptors are zeroed out on start.
If system conditions prevent the card from loading the firmware without
touching all the descriptors in the ring, it's possible for
wctc4xxp_cleanup_descriptor_ring to attempt cleanup of a command that was never
submitted resulting in the following BUG.

eab20e05cb Remove Makefiles that are only needed on kernels < 2.6.9
Newer versions of kernel build system do not require these Makefiles and
support for kernels older than 2.6.9 are no longer supported by DAHDI.

fc1824fd3c Remove support for kernels < 2.6.18
This patch removes support for kernel versions < 2.6.18, as those are
not actively supported in any major Linux distribution (except RHEL4,
which is in the "extended" support level of the product life cycle).

This removes much of the more #ifdef-rich parts of the code.

35ffe2f550 dahdi: Move WARN_ON_ONCE from wctc4xxp driver to include/dahdi/kernel.h
I only generally test on RHEL 4 when testing against kernels older
than 2.6.18. Apparently OpenSUSE 10.1 runs with 2.6.16 and doesn't
have WARN_ON_ONCE backported. I took the patch Richard Miller
originally attached to the issue and moved it to
include/dahdi/kernel.h so it would be available for all the board
drivers in the future.

4edd8a1973 wctc4xxp: Allow G723 SID frames to pass to the hardware decoder.
The driver has, until now, dropped G723 SID frames even though the
firmware on the TC400/TCE400 can handle them. Now let them on
through.

eda8ceea1c wctc4xxp: Fix lock imbalance in wctc4xxp_watchdog.
r10082 "wctc4xxp: Cleanup in-flight commands when halting due to
hardware error." introduced a lock imblance on the error path where the
cmd_list_lock would be unlocked twice when the board is halted due to a
hardware error. Thanks sparse.

d401ad4233 wctc4xxp, wcte12xp, wctdm24xxp: Remove check for HAVE_NETDEV_PRIV
DAHDI currently supports kernels >= 2.6.9. netdev_priv() has been in the
mainline kernel since versions 2.6.6 so it's available in all the
supported kernels. This change is needed to compile against the 3.1 kernel.

9fd43e0139 wctc4xxp: Cleanup in-flight commands when halting due to hardware error.
On one system I was seeing the board reset in the middle of a
transaction. Any commands that were on the response list when this would
happen would never be completed and the process would then be stuck in
an uninterruptible sleep. This change also prevents the driver from
sleeping in timer context, which would result in a kernel panic.

This change at least lets an error message propogate back to the user.

86519d1211 Remove unused variables.
gcc-4.6 now warns about variables that are set but never used. Clean up
unused variables everywhere except the oct612x subdirectory.

The oct612x should go in a separate patch in case that needs to be
pulled out into a separate project again.

25a8bb1abe "struct pci_device_id[]" -> "DEFINE_PCI_DEVICE_TABLE"
2.6.25 added the DEFINE_PCI_DEVICE_TABLE macro to make sure that the
pci_device_id tables are put into the correct section in the binary.
Convert all the places where the tables were defined to use them.

This is linux-2.6 commit where the change went in along with the
rationale:  90a1ba0c5e39eeea278f263c28ae02166c5911c8

9292e6f0ad wctc4xxp: Make the "mode" module parameter read-only in sysfs.
The mode module parameter is only acted on during module initialization.
We shouldn't allow anyone to change it after the driver is already
loaded.

37097411ea Remove mutex emulation
Using semaphores as mutexes was removed from the kernel in 4882720b267b.
Just use straight semaphores now.  'DECLARE_MUTEX()' -> 'DEFINE_SEMAPHORE()'
and 'init_MUTEX()' -> 'sema_init()'.

8d29ecb783 wctc4xxp: 'WARN_ON(1)' -> 'dev_info(...' on channel creation failure.
If the DSP ever finds it's way into a bad state it can be possible for a
timeslot to get 'wedged'.  All attempts to create a channel in a wedged
timeslot will fail and the WARN_ON macro can quickly fill the logs.

This change adds some more debugging information (which timeslot is wedged)
along with the result code from the DSP and puts it all behind a
printk_ratelimit.

b4170d8f40 wctc4xxp: Drop 'DTE_PRINTK' macro in favor of standard 'dev_xxx'
Just use the standard way to print messages associated with a particular
device. Also dropped the "Attached to device at" message since that
information is now printed on every print.

8ac8b90bd4 wctc4xxp: Remove 'service_dte' function and a few inline keywords.
'service_dte' was only called in one place and is only two lines.  I do
not think it is adding much anymore. Also, let compiler should decide if
'service_rx_ring' and 'service_tx_ring' should be inlined.

bf3fe05dfb wct4xxp: Moving the transmit short detection behind debug module param.
This needs some more testing before it's on by default.  If the card is
otherwise functioning, these messages may be confusing to the user.  If
the card is not functioning, the driver can be reloaded with debug to
check for this condition.
